Warrant Reference Request - Excel doc
  1. Save a copy of the form to your hard drive or your desktop.
  2. Fill out the form - You only need to fill out the top portion. The bottom portion will fill in automatically.
  3. Then do either of the two following:

Print and send one copy (two forms on one sheet) to the following address:

  • State Treasurer's Office
  • Attn: Treasury Management
  • Room 2003, State Capitol Bldg
  • PO Box 94788
  • Lincoln, NE 68509-4788
